CT Contrast Injection Protocol: Programming and Standardising

Updated 14 October 2026 · 4 min read

A ct contrast injection protocol defines how contrast and saline are delivered to the patient: how much, how fast, in what phases and how the scan timing relates to the injection. This article explains how these protocols are built and stored on the injector, how bolus tracking and test bolus fit in and how departments standardise them for a consistent workflow. It is about equipment and workflow only; clinical values and patient dosing are decided by the radiologist.

What a protocol contains

A protocol is a recipe made of phases. Each phase specifies a medium (contrast or saline), a volume and a flow rate, and optionally a delay or a mix of the two media. A typical protocol has a contrast phase followed by a saline flush, and more complex examinations add extra phases. The contrast injection rate for CT is therefore only one of several parameters the injector has to hold.

The injector enforces the limits that the equipment can deliver, such as maximum pressure and container volume. For the ulrich medical CT motion, with system pressure up to 17 bar, the touch terminal warns when a programmed combination of flow and line resistance would exceed the limit. Our pressure article explains this in detail.

Name protocols so that the technologist can pick the right one quickly: examination type, phase structure and any special condition, for example a limited-access variant. Clear naming prevents the most common error in this area, selecting a similar but wrong protocol under time pressure.

Bolus tracking and test bolus

Two methods link the scan timing to the individual patient. In bolus tracking CT, the scanner monitors the enhancement in a region of interest and starts the diagnostic scan when a threshold is reached. In a test bolus CT approach, a small preliminary injection is used to measure the arrival time, which then sets the delay for the main injection and scan.

Both rely on a reliable link between injector and scanner. A good protocol records which method it uses, what delay applies and whether the saline chaser is included. The injector does not decide the timing method; it executes the sequence that the protocol and the scanner define.

  • Bolus tracking: scan starts when enhancement reaches a threshold in a region of interest
  • Test bolus: small preliminary injection determines the arrival time
  • Fixed delay: used where protocol sets it explicitly
  • In all cases the saline flush is part of the programmed sequence

CT angiography injection protocol and multiphase studies

A ct angiography injection protocol usually calls for a high flow rate and a saline chaser, and cardiac or multiphase studies may add a mixed phase or a second contrast phase. Dual-flow delivery, where contrast and saline run together, is available on injectors with two channels; see the dual-head article.

Choose an injector that can store enough protocols and phases for your case mix, and that lets you copy and adjust protocols without retyping every field.

Standardising protocols across the department

Standardisation reduces errors. Define a protocol library with agreed names, restrict who may change it and review it periodically with the radiologists. On injectors with the SYNCopen option, protocols are entered once and are shown on the CT console, with injector and scanner starting together with one touch. This avoids typing the same values on two devices and cuts trips between rooms.

SYNCopen is based on the international CANopen standard and exchanges injection parameters and protocols in real time. It is available for CT motion and CT motion SPICY and can be switched on and off easily. For compatibility questions see the scanner interface article; the SYNCopen page has the specification.

Document the change control as well: who proposed a change, who approved it and when it was applied on each injector. If you have several injectors, make sure all carry the same library version.

Documentation and support

Protocols should be documented and the delivered volumes recorded; with the RIS/PACS Interface, a contrast media dose report can be sent to PACS or a dose management system. Beckfinity can support protocol set-up training when installing ulrich medical injectors in the UAE; contact our team for a consultation.

Frequently asked questions

Who decides the injection rate and volume?

The radiologist, according to the examination and local protocol. The injector only executes and enforces its technical limits.

What is bolus tracking in CT?

A method where the scanner monitors enhancement in a region of interest and starts the scan when a threshold is reached.

What is a test bolus?

A small preliminary injection used to measure how long contrast takes to reach the target vessel, which then sets the delay for the main scan.

Can protocols be sent from the scanner to the injector?

With SYNCopen on CT motion or CT motion SPICY, injection parameters and protocols are exchanged in real time between scanner and injector.
