Mayfield Retractor Systems: Types of Brain Retractors Compared

Updated 14 October 2026 · 4 min read

The term Mayfield retractor is widely used for a self-retaining brain retractor system that mounts on a Mayfield-type skull clamp, even though the retractor is a separate device from the clamp. Surgeons and buyers search for it when they want a way to hold retractor blades on brain tissue without an assistant. This guide explains the main types of neurosurgical retractor systems, how they mount and what to check before choosing.

What a Mayfield retractor is and why it is used

A brain retractor holds a thin blade against brain tissue to open a corridor to a deep lesion. Hand-held retraction ties up an assistant and moves; a self-retaining brain retractor stays where it is placed. A Mayfield retractor in common usage is a retractor system that attaches to the rail of a three-point skull clamp, so the head, the clamp and the retractor form one rigid structure.

The surgeon decides on retraction method, blade size and the pressure allowed in each case. The equipment side is to provide stable fixation, fine adjustment and fast set-up in the sterile field. For the clamp itself see the Mayfield head holder explained.

Types of neurosurgical retractor systems

Systems fall into three groups by mounting. Table-mounted systems, for example the Leyla bar family, clamp to the table rail and hold flexible arms over the head. Skull-clamp-mounted systems attach to the clamp, as the Quick-Rail on DORO clamps does. Free-standing or ring-type systems form a ring around the operative field and hold arms and blades from there.

Each type trades off stiffness, access and compatibility. Clamp-mounted systems move with the head if the table is tilted; table-mounted systems need adaptors that suit your table. Ask which mounting best fits your most frequent approaches.

  • Table-mounted bar systems (Leyla-type) with flexible arms
  • Skull-clamp-mounted systems on a rail
  • Ring-type or halo systems with blades and trays
  • Hand-held and self-retaining instruments for smaller fields

DORO LUNA, COBRA and Halo

Black Forest Medical's range includes three complementary systems. The DORO LUNA is described as its most flexible retractor and hand-rest system: six degrees of freedom, tool-free set-up in the sterile field, attachment to the Quick-Rail over several layers of drape, and ergonomic curves that can sit parallel to the floor to rest the surgeon's hands. It comes in Standard and Compact versions.

The DORO COBRA provides flexible rotary arms in four lengths with a 360-degree rotational body, tool-free mounting and micro adjustments without losing arm tension; it can sit on the Quick-Rail or directly on LUNA. DORO Halo is a ring-type system with a split, angulable halo ring in Quick-Clamp and C-Clamp versions, supplied with blade sets and a sterilisation case. See DORO Halo.

Instruments and blades: what else is in the set

Retractors are only part of the picture; the blades, pattie trays and microinstruments make the system usable. REDA's neurosurgery range covers a wide range of cranial and spinal instruments, including Yasargil microinstruments, Fergusson and Frazier suction tubes and Caspar retractors in its retractor section. See REDA neurosurgery instruments.

When comparing systems, check the blade sets that are included, the range of blade widths, the sterilisation case and rack, and whether additional blades can be reordered. For the broader instrument vocabulary see neurosurgical instruments and their names.

Selection criteria and sourcing in the UAE

Compare mounting compatibility, number of degrees of freedom, ease of repositioning, drape compatibility, reprocessing instructions and spare-part availability. Ask the supplier to demonstrate set-up in the sterile field and to confirm registration of the device with the relevant UAE authority.

Frequently asked questions

What is a Mayfield retractor?

In practice it means a self-retaining brain retractor system that mounts on a Mayfield-type skull clamp rail. The retractor and the clamp are separate devices.

What is a self-retaining brain retractor?

A system with an arm and blade that stays in position once placed, so no assistant has to hold the blade. The surgeon sets the position and adjusts it as needed.

What is a Leyla retractor?

A table-mounted bar system with flexible arms that hold brain retractor blades. It is one of several mounting concepts besides clamp-mounted and ring systems.

Can DORO COBRA arms be used with DORO LUNA?

Yes. According to the manufacturer, COBRA arms attach directly to the LUNA system and also to the Quick-Rail of DORO skull clamps.
