What Is a Cranial Perforator? Uses, Handpiece and Sizes

Updated 14 October 2026 · 4 min read

A cranial perforator is a powered neurosurgical drill that creates a burr hole through the skull and stops cutting automatically once the bone has been penetrated. It is the first instrument in many cranial procedures and is used with a motor system and a handpiece. This article explains what a cranial perforator is, how the drill and handpiece work, what size options exist and what to check before specifying one.

What is a cranial perforator?

A cranial perforator is a single-purpose tool: it drills a controlled hole through the outer table, diploe and inner table of the skull. The burr hole can be the final goal, for example for catheter or monitor placement, or the starting point for a craniotome that cuts the bone flap between holes.

The key difference from a general-purpose drill is the clutch mechanism built into the perforator drill. When the bit passes through the inner table, resistance falls and the clutch disengages, which is intended to protect the dura. The exact behaviour depends on the instrument design, so ask the manufacturer how the mechanism works on the specific bit and what the instructions for use say about technique.

Cranial perforator uses

Typical cranial perforator uses include burr holes for external ventricular drains and ICP monitors, evacuation of subdural collections, biopsies and as the entry points for a craniotomy. In a craniotomy the perforator creates one or more holes, then the craniotome connects them; see our explainer on the craniotome and the overview of craniotomy steps and equipment.

The choice of procedure and technique belongs to the surgeon. From the equipment side, what matters is that the perforator, motor and foot control are matched and that the OR team knows how to assemble, test and troubleshoot them before the patient is in the room.

Cranial perforator drill, handpiece and motor

The system has three levels: a motor or control unit, the cranial perforator handpiece, and the single-use or reusable drill bit. The NOUVAG Cranial Perforator is a handpiece for burr holes that runs at 80 to 1,200 rpm with peak torque of 120 Ncm. It connects to the motor via an INTRA (ISO 3964) coupling and accepts instruments with a Hudson coupling.

Perforator bits for this handpiece lock automatically, come in four diameters and two milling depths (1.5 mm and 3.0 mm), and are designed for single use, so there is no cleaning, reprocessing or resharpening. The NOUVAG Cranial Perforator is made in Switzerland by NOUVAG. Low speed with high torque is typical for perforators, while craniotomes run much faster.

  • Speed range 80 to 1,200 rpm
  • Peak torque 120 Ncm
  • Motor coupling INTRA (ISO 3964), instrument coupling Hudson
  • Four diameters, milling depths 1.5 mm and 3.0 mm

Cranial perforator size and bit selection

Cranial perforator size is chosen by the intended hole: a smaller diameter for a catheter or sensor, a larger one when an instrument or a craniotome footplate must enter. Milling depth must suit the patient's skull thickness, which varies with age and anatomy; two depth options allow the surgeon to select the better match.

When ordering, request the full list of bit diameters, the packaging unit and the sterile shelf life. If you plan to move toward single-use bits, our article on disposable perforator drills explains the trade-offs against reusable bits.

Buying and supporting cranial perforators in the UAE

Check that the motor system you already own has the matching INTRA coupling, that a foot switch is included, and who services the handpiece. Confirm what accessories ship in the first order and how consumable bits are replenished. Registration of surgical devices in the UAE goes through the relevant authority, so confirm current requirements with your supplier.

Frequently asked questions

What does a cranial perforator do?

It drills a burr hole through the skull and is designed to stop cutting automatically once the bone is penetrated. The hole may be used directly or as the entry point for a craniotome.

What speed does the NOUVAG cranial perforator run at?

According to the manufacturer it runs at 80 to 1,200 rpm with peak torque of 120 Ncm, via an INTRA (ISO 3964) motor coupling.

Are cranial perforator bits single-use?

The NOUVAG perforator drill bits are designed for single use, which removes cleaning, reprocessing and resharpening. Other systems may use reusable bits, so check the instructions for use.

What is the difference between a perforator and a craniotome?

A perforator makes round burr holes at low speed and high torque. A craniotome runs at much higher speed with a dura protector to cut the bone flap between the holes.
