Skin graft mesher guide for burn and plastic surgery units

Updated 14 October 2026 · 4 min read

A skin graft mesher is a device that perforates a split-thickness skin graft in a regular pattern so that it can be stretched to cover a larger wound than the harvested area. It is a standard item in burn centres and plastic surgery theatres. This guide explains what a skin graft mesher device does, how the process works and what a hospital should check when it buys one.

What is a skin graft mesher?

When a surgeon harvests a split-thickness skin graft with a dermatome, the donor area is limited, especially in extensive burns. A mesher converts the harvested sheet into a net: the graft is placed on a carrier plate and passed through a knife roller that cuts a regular pattern of slits. When the graft is stretched, the slits open and the mesh covers an area larger than the original piece.

The NOUVAG Mesher is a skin expansion system for plastic and burns surgery working on this principle. It reduces the need for additional donor-site harvesting, and the open spaces between the meshed graft close as the wound heals. See meshed split-thickness skin grafts for the surgical background.

How a mesh skin graft is produced

The sequence is simple in outline: the graft is harvested with a dermatome, laid on the carrier with the dermal side as specified by the instructions, passed through the mesher, then lifted, expanded and applied to the wound bed. Each step has details that depend on the device and on local surgical technique, so the team should be trained on the exact model.

The expansion factor is set by the cutting pattern of the carrier or cutter, with ratios in common clinical use ranging from small expansions up to wide meshes. Higher expansion covers more area but leaves larger open spaces that heal by epithelialisation. Our article on how to use a skin graft mesher goes through the steps in more detail.

Manual and motorised skin graft mesher devices

Meshers are available as hand-cranked units and as motorised systems. The hand-cranked design is mechanically simple, quiet and independent of power. Motorised systems add speed and consistency in busy theatres. Whichever you choose, the roller must be easy to remove for cleaning, because retained tissue is the main reprocessing challenge.

For skin harvesting itself, hospitals pair a mesher with an electric dermatome. NOUVAG's TCM 3000 BL motor system drives the NOUVAG Dermatome with cutting widths of 75 or 100 mm and an adjustable cutting depth from 0.05 to 1.0 mm. Read more in our overview of electric dermatomes for skin grafting.

What to check when buying a mesher

Ask which carrier plates and expansion ratios are available and what they cost to replace, because carriers are a recurring part of the budget. Check which components are reusable, how the knife roller is removed for cleaning, and whether the instrument can be steam sterilised according to the instructions. The NOUVAG Mesher has a removable knife roller for maintenance and cleaning and measures 210 × 195 × 110 mm with a weight of 4.9 kg.

Also check handling: how the graft is loaded, how easily the carrier is changed and how the device fits on a trolley or table. Review availability of spare parts, sharpening or replacement of the knife roller and service response in the UAE. Our article on carriers for skin graft meshers covers this in more depth.

Frequently asked questions

What does a mesh graft device do?

It cuts a regular pattern of slits in a split-thickness skin graft so that the graft can be stretched and cover a larger wound area than the harvested piece.

What is a skin graft mesher used for?

Burn surgery and plastic and reconstructive surgery where donor skin is limited and a large wound has to be covered.

Is a mesh considered a graft?

A meshed graft is a split-thickness skin graft that has been perforated and expanded. It is still an autograft; the mesh describes how it is prepared.

Can the mesher knife roller be cleaned?

On the NOUVAG Mesher the knife roller is removable for maintenance and cleaning. Follow the instructions for use for reprocessing.
