Digital radiography and PACS: DICOM services and integration steps

Updated 14 October 2026 · 4 min read

Digital radiography and PACS work well together only when the DICOM connection between them is planned, configured and tested properly. A DR system that exports images but ignores the worklist still forces staff to retype patient data, and that is where errors start. This guide explains the DICOM services to ask for, the steps of a typical integration and the pitfalls we see most often, for hospitals and clinics in the UAE.

The DICOM services a DR system needs

When you ask whether a DR system supports DICOM, the answer should be a list of services, not a yes. The services that matter in daily work are: Modality Worklist (MWL) to receive patient and order data from RIS or HIS; Storage to send images to PACS; Storage Commitment to confirm that PACS has safely stored them; Modality Performed Procedure Step (MPPS) to report that an exam started and finished; and a radiation dose structured report (RDSR) or equivalent for dose tracking.

Ask for the DICOM Conformance Statement of the exact software version. It lists supported services and roles and is the document your IT team and PACS vendor will work from.

Worklist: the DICOM worklist removes retyping

With a DICOM worklist, the radiographer selects the patient from a list sent by RIS rather than typing demographics. This reduces misspelled names and wrong IDs, and it keeps accession numbers consistent so that images and reports match. Make sure the system can query by date, modality and station, and that it handles emergency patients without a prior order using an agreed temporary-ID workflow.

The same logic is used in other imaging equipment. For example, the ulrich medical RIS/PACS Interface lets staff select the patient from the DICOM worklist on the injector's touch terminal and sends a contrast dose report to PACS automatically, which shows how worklist and report flows reduce manual documentation. See the product page for its published details.

Integration steps from planning to go-live

A typical sequence is: collect conformance statements from the DR vendor and the PACS/RIS vendors; agree AE titles, IP addresses, ports and the network segment; configure the worklist and storage destinations; test with sample patients covering normal, emergency, paediatric and repeat exams; verify that images appear in PACS with the correct patient, study description, orientation and laterality; then train staff and run in parallel for a short period before switching over.

A modern modular room such as the Villa Moviplan iC DR works with VDX Next acquisition software that provides full DICOM integration; confirm with the supplier which services are licensed in your configuration.

Common pitfalls with DR systems and PACS

The most common problems are mismatched patient IDs between HIS, RIS and PACS, wrong or missing character sets for Arabic names, clocks that are not synchronised, firewall rules that block the DICOM port, rejected images that are still sent, and annotations burned into the image. Another frequent issue is dose reporting that is switched off because nobody was assigned to set it up.

Treat cybersecurity as part of the project: restrict network access to the modality, apply updates according to a plan agreed with the manufacturer, and document who may change the DICOM settings.

What to put in your specification

Write the DICOM services as requirements, require the conformance statement and a factory or site acceptance test with your PACS, and make payment milestones depend on successful integration. The article on X-ray equipment tender specifications shows how to structure such requirements, and the overview of best practices in digital radiography covers the workflow after go-live.

Beckfinity specialists can coordinate with your IT team and PACS vendor on DR integration for the equipment we supply.

Frequently asked questions

What DICOM services should a DR system support?

At least Modality Worklist, Storage, and preferably Storage Commitment, MPPS and dose reporting such as RDSR.

Why use a DICOM worklist?

It sends patient and order data from RIS to the modality, reducing retyping errors and keeping images matched to reports.

What document do we need from the vendor?

The DICOM Conformance Statement for the exact software version you are buying.

What often goes wrong during integration?

Patient ID mismatches, character sets for Arabic names, time synchronisation, firewall rules and unconfigured dose reporting.
