Morcellator for HoLEP: Principles, Components and Selection

Updated 14 October 2026 · 4 min read

Every morcellator HoLEP programme depends on the same step: after the laser has enucleated the prostate adenoma into the bladder, the tissue must be fragmented and removed quickly and safely. This article explains why HoLEP needs a dedicated morcellator, how a urology morcellator scope works and which features hospital buyers in the UAE should compare.

Why a morcellator is part of the HoLEP workflow

In holmium laser enucleation of the prostate, the surgeon separates the adenoma from its surgical capsule and pushes the lobes into the bladder. Enucleation is therefore only half of the operation. The detached tissue is too large to pass through the sheath and has to be broken into pieces and aspirated.

The question of morcellation vs enucleation is often raised, but they are consecutive steps, not alternatives. Enucleation frees the tissue; morcellation removes it. Speed and safety of the second step affect total operating time and the risk of bladder mucosal injury, which is why a dedicated morcellator is a standard part of an endoscopic enucleation set-up.

How an endourologic morcellator scope works

A urology morcellator consists of a nephroscope-like morcellator scope, a cutting blade that rotates inside a hollow tube, and a suction pump. Tissue is drawn to the blade window by suction, cut and carried out through the lumen into a collection container. The surgeon keeps the window directed at tissue and away from the bladder wall.

A foot switch controls cutting and aspiration. Fluid management matters: the bladder must be filled enough to protect the wall, while irrigation and suction balance keep the view clear. Training and a stable, repeatable technique reduce complications; the decisions remain the surgeon's.

The Richard Wolf PIRANHA system

The Richard Wolf PIRANHA is a morcellation line for endoscopic enucleation of the prostate and is designed to work with SHARK resectoscopes. The package consists of the PIRANHA morcescope, a suction pump set and the Vmax blade, which is available in disposable and reusable versions and runs at 1,500 rpm.

Specific features include a morcescope with two irrigation channels and a 5 mm working channel, a telescope that fits 24 Fr and 26 Fr outer sheaths, a SNAP-ON coupling with an automatic valve against water loss, and a footswitch that toggles between PIRANHA mode and suction mode. The suction pump has a separate container so that tissue remains suitable for histology.

What urology buyers compare in a morcellator urology system

Compare blade options and cost per case, since a disposable blade removes sharpening and reprocessing while a reusable one shifts work to sterile services. Check sheath compatibility with the resectoscopes you already own, how the system keeps the bladder distended, and how easily tissue can be collected for histology.

Also assess the ergonomics of the handpiece, the noise of the suction pump, the number of reusable components and the instructions for cleaning the lumen. Our general article on the tissue morcellator covers technology that is common to both gynaecology and urology, and what is tissue morcellation explains the technique.

Service, training and regulatory points

Ask whether training for surgeons and theatre staff is included, how spare blades are supplied in the UAE, who services the pump and what the response time is. For a laser enucleation programme, the morcellator should be considered together with the laser, resectoscope and fluid management equipment.

Medical devices are registered with the relevant authority, and facility licensing is handled at emirate level by bodies such as DHA or DOH. Confirm current requirements before ordering.

  • Disposable versus reusable blade policy.
  • Training, installation and service route in the UAE.

Frequently asked questions

Why is a morcellator needed in HoLEP?

After enucleation the prostate tissue lies free in the bladder and is too large to remove through the sheath, so it is fragmented and aspirated by a morcellator.

Is morcellation vs enucleation a choice?

No. Enucleation separates the adenoma; morcellation removes it. They are consecutive steps in the same operation.

Can morcellated prostate tissue be sent for histology?

In the PIRANHA system the suction pump uses a separate container so that tissue remains suitable for histology.

Are blades disposable or reusable?

The PIRANHA Vmax blade is available in both disposable and reusable versions.
